We will assist you in building the skills you need to understand and use your plan. A support coordinator will work with you to ensure a mix of support is used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in your community.

   We offer free consultation to help you understand the NDIS and guide you through the application process.
If needed, we're more than happy to meet with you in person to make the journey clearer and more comfortable.

We help participants diagnosed with psychosocial disability to access specific services that they may need. Especially, Psychosocial Recovery Coaches work with the Care team and participants to develop their personalized Recovery and Crisis Management Plan. 


The Recovery and Crisis Management Plan is a plan that assists participants in building up their skills to cope with the crisis that occurred due to the psychosocial disability conditions that the participants have. 


Our team members with mental health knowledge & training will actively listen and work through the crisis with the participant. 


We aim to look for the best way to assist the participants in establishing their capability to deal with crisis.

Level 1 - Support Connection: Helps you connect with paid and community supports and build confidence in using your plan. 


Level 2 -Coordination of Supports: Actively coordinates multiple services and supports to support your goals


Level 3 - Specialist Support Coordination: Provides intensive assistance for complex situations or risks.

Why Support coordintor? What Is a Local Area Coordinator (LAC)?

 

A Local Area Coordinator (LAC) is someone who works for a partner organisation of the NDIS (not the NDIS itself). Their role is to help you:

  • Understand the NDIS and apply for access
     
  • Set goals during your planning meeting
     
  • Connect with mainstream and community services
     
  • Review and reassess your plan with the NDIA
     

LACs provide general support and work with a wide range of participants, especially those with Level 1 (Support Connection) funding or no funded Support Coordination.

How Support Coordination Is Different

Support Coordination is a more intensive, personalised service that is funded in your NDIS plan (usually under Levels 2 or 3). At Care & Connection, we offer professional Support Coordination to help you:

  • Understand and implement your funded supports
     
  • Coordinate multiple service providers
     
  • Navigate complex or changing situations
     
  • Build your capacity and confidence to manage your own supports
     
  • Advocate for your needs and resolve service issues
     
  • Prepare for plan reviews with clarity and support

1. Plan Review & Goal Setting

We start by reviewing your NDIS plan together. We take the time to understand your goals, needs, and preferences — ensuring you feel heard and supported from day one.

2. Identifying Barriers & Tailored Supports

We help identify any challenges or gaps in your current supports and work with you to find the right solutions.


As a first step, we’ll communicate with your existing care team to explore whether improvements can be made within your current supports — helping you avoid unnecessary disruptions or major changes in your life.


If needed, we can also assist in connecting you with culturally responsive services, specialist providers, or local community supports that better align with your needs and goals.

3. Progress Monitoring & Plan Adjustments

We don’t just set goals — we stay actively involved in helping you achieve them.
We work closely with you and your support team to ensure everything is running smoothly, and through regular check-ins, we monitor your progress, review what’s working, and make adjustments where needed. Our focus is to keep your supports aligned with your changing needs, so you feel empowered and supported at every stage. 

4. Crisis Support & Complex Coordination

For participants with more complex needs, we offer responsive support during times of crisis or instability. We work closely with allied health, housing, mental health, and clinical teams to ensure coordinated, safe, and timely interventions. 

5. Capacity Building at Every Stage

Our approach is focused on building your skills and confidence over time — so you can gradually take more control over your supports, decisions, and future planning.
